Think it happens a lot in the winter. People start the gas and go back into their running / heated car. It’s a bad idea because you can generate static electricity going in and out of car, pump could start to overflow, and well… you should be watching pump when something highly flammable is being pumped into your vehicle.
Fun fact: Formula 1 actually banned refueling because they had so many accidents by doing it. The cars start the race with enough fuel to run the whole thing.
According to myth busters, smoking in and of itself is also not a problem when pumping gas. Cigarettes don’t burn hot enough to light gasoline. A lighter CAN light gasoline tho (obviously).
I’m not sure I’d say it’s completely harmless, but isn’t pumping gas while the car is running generally not a huge risk?
Why would anyone do that outside of a formula one race?

I saw my high school chemistry teacher do it and right then, I knew it had to be safe.
